GOFA SilverCare
Preventive Elderly HealthCo-creation Pilot Programme
Bring AI preventive health into the community
Starting with fall-risk and functional health screening, the programme helps communities identify risks earlier, intervene earlier and track outcomes over time.
GOFA SilverCare aims to work with NGO and professional partners to build a scalable preventive elderly health service model for Hong Kong communities.
Why now
Preventive elderly health is moving faster into the community
Hong Kong is expanding district health and elderly health services. More older adults will receive health assessment and ongoing health management in community settings.
The real challenge is not only completing more assessments, but making the whole service pathway work with limited professional manpower.
Policy context is for reference only and does not imply endorsement, adoption or partnership by the Government, PHCC or DHC.
Start with the clearest problem
Begin with fall risk and Functional Health
Many community elderly centres may not have a physiotherapist onsite long term, making functional health and fall-risk screening difficult to run frequently and at scale.
GOFA SilverCare uses AI to help frontline teams complete preliminary functional health screening, identify older adults who need priority follow-up, and connect them to suitable community intervention or professional services.
Co-creation Pilot Programme
Run the model in real community settings with partners
GOFA SilverCare is inviting a small number of NGOs to become the first Founding Implementation Partners.

Want to explore a pilot at your service point?
Arrange a 30-minute management briefing to discuss how to begin with fall-risk and functional health screening.
Professional and community advisors participate in their personal capacity. Their involvement does not represent endorsement, adoption or partnership by any current, former or affiliated organisation. GOFA SilverCare supports community preventive health and functional health service workflows; it does not replace clinical judgement, diagnosis or treatment by doctors, physiotherapists or other registered professionals.
Our direction
Start with fall risk, then build toward a fuller community preventive health platform
Phase 1 focuses on fall risk and Functional Health. Once the model works, it can extend gradually to other elderly health domains according to community needs and professional frameworks.
